What are the out-of-pocket costs typically associated with the broader coverage provided by major medical plans?

While major medical plans do provide coverage for a wide range of medical expenses, insureds under such plans are expected to pay some costs out of their own pocket. Included in these costs is the deductible, which must be satisfied before expenses are covered, as well as, any costs that are insured as a result of a co-insurance clause.

Also keep in mind that in order to be covered by the plan, medical costs must be judged “reasonable and customary” as defined by your insurance company. Many factors are considered when making this judgment, including residential geographic location, and can vary depending on the company with which you are doing business.
